If you are a new patient, please fill out the online pre-registration in advance of your appointment to assist the staff in making sure that we have all the information necessary to provide you with quality care and treatment The online pre-registration includes your medical history and is referred to as One Medical Passport. Please familiarize yourself with the consent forms, you will be required to sign them on your day of surgery. It is not necessary to print the consent forms or the One Medical Passport.
